Scope of Appointment

Before any appointment where Medicare Advantage or Part D plans will be discussed, a Scope of Appointment (SOA) form is required by CMS. Janelle will send you an SOA to complete prior to your consultation — this simply documents the topics you'd like to cover and is a standard part of the Medicare planning process.
